1. What Is Protected Health Information
Protected Health Information (PHI) is any information that identifies you and relates to your past, present, or future physical or mental health, the care you receive, or payment for that care. This includes information you share when you request a consultation, verify insurance, or receive treatment.
2. How We May Use and Disclose Your PHI
HIPAA permits us to use and disclose your PHI, without additional authorization, for the following purposes:
- Treatment: To provide, coordinate, and manage your care with the clinicians and staff involved in your treatment.
- Payment: To verify insurance benefits, obtain prior authorization, and bill for the care we provide.
- Health Care Operations: To support quality improvement, staff training, care coordination, and other operational activities.
We will obtain your written authorization before using or disclosing your PHI for any purpose that requires authorization under HIPAA, including most marketing communications and any sale of PHI. You may revoke an authorization in writing at any time, except to the extent we have already acted on it.
3. Other Permitted or Required Disclosures
We may use or disclose your PHI without authorization when permitted or required by law, including for public health activities, reporting abuse or neglect, health oversight activities, legal proceedings, law enforcement purposes, to avert a serious threat to health or safety, and to comply with federal, state, or local law. Certain information, such as substance use disorder records, may receive additional protection under federal and state law, and where a stricter law applies, we follow the stricter requirement.
4. Your Rights Regarding Your PHI
You have the right to:
- Access and receive a copy of your health record.
- Request corrections or amendments to your health information.
- Request an accounting of certain disclosures we have made.
- Request restrictions on how we use or disclose your information.
- Request confidential communications by an alternative means or location.

- Be notified in the event of a breach of your unsecured PHI.
If you pay out of pocket in full for a specific service, you may request that we not disclose PHI about that service to your health plan for payment purposes.
5. Our Responsibilities
We are required by law to maintain the privacy and security of your PHI, to notify you following a breach of unsecured PHI, to follow the terms of the notice currently in effect, and to provide you with information about our privacy practices. We reserve the right to change this notice, and any changes will apply to information we already hold as well as information we receive in the future.
